This article presents the results of a prospective, randomized, double-blind, placebo-controlled, crossover study of SEROQUEL XR, a medication that, in combination with the standard anti-psychotic drug quetiapine, has been shown to decrease the incidence of hospitalization for major depressive disorder (MDD) in patients with a history of treatment-resistant MDD.

In this study, a randomized, placebo-controlled, double-blind, parallel-group trial (NCT01693345) of quetiapine was carried out among 10 consecutive patients with a history of MDD (i.e., at least 1 previous episode of treatment-resistant depression) and with a history of treatment-resistant depression. Patients were randomly divided into two groups. The first group was administered quetiapine (1 mg/d;n=10), and the second group was given quetiapine (50 mg/d;=10). In the quetiapine group, the incidence of hospitalization was significantly reduced as compared to the placebo group at week 12, at week 24, and at week 46 (at week 56).

The primary efficacy endpoint was the incidence of major depressive episode in the quetiapine group. There was no significant difference between the quetiapine group and the placebo group regarding the percentage of patients who reported that they had taken quetiapine and quetiapine-containing medications within the previous six months. In addition, there was a significant reduction in the rate of major depressive episode in the quetiapine group as compared to the placebo group (P<0.01).

The most significant adverse events in the quetiapine group were headache (26%), nausea (13%), dizziness (7%), and diarrhea (6%).

In conclusion, quetiapine was associated with a significant decrease in the rate of major depressive episode and the incidence of major depressive episode in patients with a history of treatment-resistant depression. Therefore, quetiapine should be used as first-line therapy for treating patients with MDD.
